Account Deletion

Request deletion of your Atulya clinician account. Because Atulya is a HIPAA-regulated platform used by hospice organizations, account deletion is coordinated with the Customer Organization that provisioned your access.

Atulya is a HIPAA-regulated platform used by hospice organizations. Clinician accounts are provisioned and managed by the Customer Organization that employs you.

To delete your Atulya account

Submit a request using the form below, or email support@atulya.health with the subject line "Account Deletion Request." Include your name, work email, and the name of your organization.

We will coordinate with your organization to deactivate your account and delete personal information that is not subject to a legal or contractual retention obligation. Most accounts are deactivated within 14 days; complete deletion of associated audit logs and other records may take up to 6 years to comply with HIPAA's retention requirements.

Patient health information is owned and controlled by the Customer Organization, not by Atulya, and is governed by the Business Associate Agreement between Atulya and the organization. Patient information cannot be deleted through this request. Patients seeking to exercise rights regarding their health information should contact the hospice or healthcare provider directly.
